Application for registration of Para Medical can be obtained from the
office of State Medical Faculty located at
:. Directorate of Medical Health & Family Welfare, 107,
Chander Nagar, Dehradun.:

Application to be submitted along with the following

Check List for Registration
  1. Rs. 500.00 Bank draft, in favour of  Secretary, Uttarakhand, State Medical  Faculty, Dehradun, payable to State Bank of India, Kacheri
  2. Self attested four colored passport size photograph
  3. Four specimen  signature at plain paper
  4. Domicile certificate
  5. High school, Intermediate mark sheet and self attested photocopy of pass out Paramedical        diploma
  6. Self attested photocopy of diploma certificate
  7. Two self attested copy of original certificate of UP registration
  8. Pledge certificate on stamp paper of Rs. 10.00
  9. Certificate related with Planning/employment
  10. Two envelop 10x12 one plain & one with RS. 30 Stamp
  11. One file cover with tag
  12. Application for removing the name from State Uttar Pradesh Faculty.


Application for registration of Staff Nurse/ANM can be obtained from the
office of  State Medical Faculty located at
Directorate of Medical Health & Family Welfare,  107, Chander Nagar, Dehradun.

Application to be submitted along with the following

Check List for Registration

  1. Photocopy of High School/Intermediate Mark sheet & Certificate
  2. Photocopy of Diploma Mark sheet &  Certificate
  3. Certificate of Uttarakhand domicile/work experience certificate of working in Uttarakhand
  4. Three colored passport size photograph
  5. Rs. 30.00 stamp with Self addressed 10x12 inches envelope
  6. File Cover with tag
  7. Draft for registration fee, which is  payable to the Registrar Uttarakhand Nurses Midwives Council, Dehradun payable at State Bank of India, Kacheri Branch (Branch Code No- 7893) Dehradun.

Registration Fees:

  1. ANM for Rs. 300
  2. Staff Nurse for Rs. 550

In condition of change in name Rs. 110.00 amount to be added with the above fee. 
The fees shall be:
  1. ANM for Rs. 410
  2. Staff Nurse for Rs. 660                        


Note : It is compulsory that applicant is present with the enclosed original certificate
